About Traumatic Brain Injuries (TBIs)
TBIs affects 2.8 million US people per year with the majority (70%) of cases being mild. TBIs occur when external mechanical force causes brain dysfunction, ranging from mild concussion to severe diffuse axonal injury. TBIs disrupt neuronal integrity, neurotransmitter balance, and cerebral blood flow, classified by Glasgow Coma Scale (GCS 13-15 mild, 9-12 moderate, ≤8 severe) and duration of loss of consciousness/post-traumatic amnesia.
Symptoms
- Mild TBI (concussion): Headache, dizziness, fatigue, nausea, photophobia, concentration difficulty, sleep disturbance, mood irritability (appears immediately or delayed).
- Moderate to Severe TBIs: Vomiting, seizures, unequal pupils, clear CSF rhinorrhea/otorrhea, prolonged LOC (Loss of Consciousness) >30 min or >24hr respectively, focal deficits, and post-traumatic amnesia < or > 7 days respectively.
- Cognitive: Memory impairment, executive dysfunction, processing speed reduction.
- Chronic: Post-concussion syndrome, CTE (progressive neurodegeneration). IN ACUTE INJURIES, CALL 911 AND SEEK IMMEDIATE EMERGENCY MEDICAL ASSISTANCE IN SUSPECTED TBIs. ONCE CLEARED BY THE EMERGENCY MEDICAL PERSONNEL – GET INTEGRATIVE TREATMENT IMMEDIATELY TO QUELL OR PREVENT THE LONG-TERM SECONDARY CASCADE INJURY EFFECTS.
Causes / Effects
- Primary: Direct mechanical force causing brain/nervous system dysfunction and/or injury leading to cranio-cervical instability
- Secondary Cascade Injury (Hours–Days Post Impact):
- Excitotoxicity: glutamate surge → Ca2+ influx → neuronal cell death
- Ischemia: contusion expansion and cerebral perfusion pressure drop
- Inflammation: microglial activation and blood–brain barrier breakdown
- Mitochondrial Failure: ATP energy depletion and reactive oxygen species (ROS) generation
- Apoptosis: secondary neuronal loss peaks day 3–7
Post-concussion symptoms like headache, dizziness, mood changes, cognitive fog, and disabilities that stem from this neurometabolic uncoupling can be self-limiting in mild TBIs but not in moderate to severe TBIs, and it is best to get integrated treatment immediately after being cleared by emergency medical personnel for the best and most rapid therapeutic outcomes.
